Physical Therapy Evaluation

Your first appointment will be an initial evaluation. A licensed, Physical Therapist will assess your current condition as well as discuss your plan of care. Your plan of care will be determined by a collective effort by your therapist and physician. This will also give you the opportunity to ask any questions and voice any concerns that you may have regarding your Physical Therapy. After your evaluation, the Physical Therapist will communicate with your referring physician and inform them of your plan of care.

Preparation

  • Your initial evaluation could last up to 90 minutes.
  • BSC staff will e-mail, mail, or request that you down load the New Patients form online.
  • Please fill this paperwork out prior to coming to your first visit. If for some reason you are unable to complete the paperwork prior to the first visit, arrive 30 minutes before your scheduled appointment time. Having this paperwork filled out prior to your appointment time, will allow the therapist to have as much time with you as possible to focus on your condition.
  • If your paperwork is complete, please arrive 15 minutes before your scheduled appointment time.
  • Bring your insurance cards, driver’s license, your prescription from your Physician, and any other necessary paperwork.
  • Please wear clothing that will allow us to treat the designated body part.

Return Patients

  • Any patient that has not been seen within 30 days will be considered a new evaluation.
  • You will need to update all demographic and insurance information with a front desk associate before scheduling an appointment.
  • Bring your insurance cards, driver’s license, your prescription from your Physician, and any other necessary paperwork.
  • Please wear clothing that will allow us to treat the designated body part.

Medical Fitness Evaluation

Body Structure does not have fitness club standard enrollment/initiation fees. What we do have is three different initial evaluations based on your needs and safety. A certified personal trainer will assess your current condition, help clarify goals, and establish a plan with you. Your plan of care will be determined by a collective effort between you, your personal trainer, and if necessary, your physician.

Pre-Evaluation Guidelines

IN ORDER TO ASSURE THE MOST ACCURATE RESULTS, PLEASE FOLLOW THESE GUIDELINES:

  1. It is important that you disclose any health or medical problems you may have prior to your evaluation or during your Health History Review. If you are under a physician’s care for heart disease, high blood pressure, back pain or any other serious health problem, you should get clearance from your doctor before participation in this program.
  2. Avoid any strenuous exercise within four hours of your evaluation.
  3. Avoid eating a heavy meal before your evaluation. Eating a light (low fat) breakfast or lunch two hours before the evaluation is acceptable.
  4. Avoid the use of alcohol, caffeine, tobacco, drugs or medications, within four hours of your appointment. Use of any of these substances may significantly distort your cardiovascular results (i.e. heart rate, blood pressure).
  5. Dress in athletic attire. We recommend a t-shirt, shorts, and athletic shoes. Avoid pants that cannot roll above your thigh, because they make it difficult to get an accurate skin fold measurement on the mid-thigh, which determines your percent body fat.
